The Medical Bunker is a Wehrmacht structure. In skirmishes, it is not built directly and instead, is a conversion upgrade to the .

Once a Concrete Bunker is converted into a Medical Bunker, it loses its ability to garrison troops and instead will passively heal nearby friendly squads (including teammates), while also being able to reinforce nearby squads. The Medical Bunker heals nearby infantry at a rate of 3.2 hitpoints per second. Additionally, similar to the USF , it will also create two invulnerable, non-targetable medics. These medics initially do nothing and standby, but as the player's infantry units fall in battle, the medics will move out and attempt to collect casualties to bring back to the bunker. Each casualty brought back will grant a free reinforcement to nearby friendly squads the next time they are near the bunker. Note that teammate casualties are not included, only the Wehrmacht player's own casualties are collected.

Since bunkers can be built in any friendly territory, including outside the HQ territory, this means that Medical Bunkers can also serve as a source of forward healing/reinforcement method for the player and/or their teammates. However this is generally risky as the structure is stationary and thus can be quite vulnerable to being destroyed depending on the situation.

Typically it is common for players to obtain the Medical Station upgrade at the to gain healing for their units in a match. However, since Medical Bunkers can also heal, it is possible to build one at base to serve as an alternate means of healing with a different cost and time opportunity trade-off. In particular, constructing a bunker and then upgrading it will require more time (including tasking a or to build a bunker in the first place), as well as costing Munitions instead of Fuel to upgrade. Overall though, using a Medical Bunker in place of researching the HQ upgrade is far more cost efficient due to long term Manpower savings from the casualty collection benefit.

As with any other structure, it is quite hardy against small arms but is vulnerable to artillery, anti-tank weapons, and flamethrowers.
